Kindergarten through 6th Grade 

Eligibility Requirements:

  • In order to be eligible for Kindergarten, a child must be 5 years old on or before August 31, 2012.
  • In order to be eligible for 1st Grade, a child must be 6 years old on or before August 31, 2012
  • Child must reside in Dallastown Area School District with their Parent or Legal Guardian.

Documentation Requirements:

The following documentation is required at the time of registration. Parent must obtain all of the documentation prior to the appointment, or they will need to reschedule.
 
2 Proofs of Residency (one proof is required from each list):
  • List A
    • Lease Agreement (This is Required if you are not the Owner of the Property. If you are not the owner of the property and cannot provide a lease, the owner/leaseholder will be required to attend with you. Please inform Central Registration staff when they call you to Schedule the Registration Appointment. Parent/Guardian and Children being Enrolled must appear as tenants on the Lease Agreement.)
    • Utility Bill (Acceptable as List A item only if you are the Property Owner)
    • Settlement Statement
  • List B
    • Pennsylvania Issued Driver’s License, Photo ID , or Vehicle Registration
    • Utility Bill (Cannot be used if chosen as Proof from List A)
    • Charge account bill
    • Homeowner’s or Renter’s insurance
    • Vehicle insurance
    • Letter from hotel manager
    • Change of address at place of employment
    • State/Federal program enrollment
    • Bank statement
Student’s Immunization Record.
  • 4 doses of tetanus (additional dose is necessary if 5 years have elapsed since last immunization)
  • 4 doses of diphtheria
  • 3 doses of polio
  • 2 doses of measles
  • 2 doses of mumps
  • 1 dose of rubella
  • 3 doses of hepatitis B
  • 2 doses of varicella (chickenpox) or history of disease
Student’s Birth Certificate
 
Student's last report card (for Grades 1 through 12)
 
Discipline Record (only required if student has previous record, for Grades 1 through 12)
 
Court Approved Custody Papers or Agency Placement Letter (Note: This is Required if Individual other than Natural Parent is Enrolling Student or if Student’s parents have Custody Agreement)
