Volunteer Clearances

Volunteers play a vital role in supporting the staff, students, and programs of the Dallastown Area School District. We are very thankful that you have expressed an interest in volunteering in our district.

The following documents must be on file with the District prior to the first day of volunteer service:

• Volunteer Information Sheet - Please complete and submit with appropriate documentation
• PA Child Abuse History (ACT 151) - Child Abuse clearance website
• PA State Police Criminal Check (ACT 34) - State police clearance website
• FBI Clearance Exemption Form - If you have lived in the Commonwealth of PA for 10 OR more consecutive years, you do NOT need to obtain a FBI clearance. Sign and date this form.
• FBI Clearance Website - Enter service code 1KG6ZJ

In addition to completing the proper clearances, you will also need to review the information below regarding Mandated Reporting and sign the Acknowledgement form. Once all of the above has been completed, submit to the building secretary for approval.

• Mandated Reporter Information - Review all of the frequently asked questions at the bottom of the page.
• Mandated Reporter Acknowledgement - Sign and submit after reviewing mandated reporter information.

Use only for immediate volunteer start date. This does not replace the above required documents.  

• Affidavit & Agreement - EXPIRES IN 90 DAYS.
