Upcoming Physicals!
Physicals will be offered at Dallastown High school on June 1st, 2022 for the 2022-2023 School Year. Physicals will be from 5:30 to 8:00 pm and will be $15 (cash or checks made payable to DASD.) Physical paperwork should be filled out prior to attending. You can find the physical paperwork at the bottom of the page
The OSS Health Sports Medicine team is committed to continually serving the needs of our student athletes through
this difficult time. It is our goal to accommodate as many of the fall student-athletes as possible, while keeping them

The Pennsylvania Interscholastic Athletic Association (PIAA) rules state that all students must have a new physical exam every year in order to be eligible to play a school sport. Students who plan to play any sport or do cheerleading during the 2022 – 2023 school year must have their physical exam done on or after June 1, 2022, per PIAA rule.
WHAT TO BRING: PIAA Physical Exam form, Sections 6 & 7. Please fill out Section 5 (Health History) prior to obtaining your physical (the examining physician will need to see the completed health history prior to completing the physical). The form can be found on the HS Athletics website under SPORTS PHYSICAL INFORMATION. It will also be available within each individual student-athlete’s online SportsWare account. Please turn in the completed forms to the High School Athletic Trainer prior to sport participation. During the summer, paperwork can be turned in to the HS Main Office to be placed in the Athletic Trainer’s mailbox.
NEW REQUIREMENTS FOR DASD SPORTS:
SPORTSWARE ACCOUNT: All athletes must have an online SportsWare account set up and completed. If your student-athlete is new to sport, please see full step-by-step instructions for completing this requirement on the Dallastown HS Athletics website under Sports Physical Information. If your student-athlete created this account last school year for sports, please update any changes in information, and fill out all necessary online paperwork within the FORMS tab. SportsWare accounts must be completed in entirety prior to any participation in school sports, including tryouts.
IMPACT TESTING: ALL student-athletes entering 7th, 9th, and 11th grades in the 2022-23 school year, as well as any new students, regardless of grade, to DASD sports, must take the Baseline ImPACT test prior to any participation in school sports, including tryouts. This test has now been approved to be a home-based test. Instructions for how to complete this baseline test can be found on the HS Athletics website under SPORTS CONCUSSION INFORMATION. The instructions can also be found within each individual student-athlete’s online SportsWare account under the homepage heading of HANDBOOKS AND POLICIES – titled AT HOME Baseline ImPACT Instructions.
DISCLAIMER: Please be aware that the school physician may require a student-athlete to obtain additional clearance from the family doctor or a specialist before he/she is made eligible to begin practice, should an existing health problem or recent injury/illness be present.
WINTER/SPRING SPORT SEASON ATHLETES: If you choose to have a physical completed more than 6 weeks prior to the start of your season, please be advised that a Section 8 Parent Recertification will be required prior to participation in your sport, per PIAA rule. A Section 9 Doctor’s clearance may also be required if a significant injury or illness was sustained after the sports physical was completed.
**All requirements, including paperwork and SportsWare account, are due no later than August 2, 2022. Anything completed after this date may affect eligibility, due to the processing of such paperwork. Please turn in physicals to the Athletic Trainers via the HS Main Office.**

example: If you are a current 8th grader anticipating signing up for
Junior High football (9th Grade), you should fill out the completed
paperwork as a 9th grader.

You should have received a School ID from the athletic trainer. This is required to join the correct school.

Once your request is accepted you will receive an e-mail with the Subject “SportsWare request accepted”.

Start entering Athlete’s information.
Complete the Athlete Online Access section
by updating it with the Athlete’s email address
(defaults with Parents initially). Then Update
The Parent Online Access section with email
and password. Continue to complete the remaining tabs.

Once done with My Info go to:
Med History: Complete a Medical History questionnaire.
Forms: View/complete required paperwork. Note: SportsWare will also display “You have ? forms to complete/download”.
Print: Print My Info and Medical History data.

ADDITIONAL INFORMATION:
- Each student is required to have his/her own SportsWare account, even within the same family. Each account is specific to the individual.
- It is ideal for the parent to be the administrator of their child’s SportsWare account. We utilize SportsWare to convey important information via email throughout the year.
- The same email address can be used for each student’s account; however, it is required that you use different passwords.
- When first creating your account, you will be asked to enter the School ID (dtwildcats), then input first and last name, email address, and group. Once you complete this, click SEND. This is as far as you can go until the athletic trainers accept your request. You will receive an email notification when you have been accepted. At this time, you may log in and complete your profile. Please allow up to five (5) days for an acceptance email notification.
- If you receive an incomplete message when logging out of SportsWare, please log back in to correct the missing information. Your child cannot be marked as cleared until all information is completed. This information is used by coaching staff and athletic trainers as emergency contact information.
- Please update SportsWare information as it changes. It is parental responsibility to complete this.
- Please check junk mail for SportsWare acceptance emails. Depending on personal filters, this may go to your junk mail folder.
- Due dates for the Recertification form for Winter and Spring seasons will be communicated via the email address used to log in to your SportsWare account. Again, please make sure your log in is a valid email address.
