Enroll Student(s) at DASD

General Instructions

Our desire is to guide you through the on-line process and to do so, we need you to be prepared. The following documents are REQUIRED to complete every student registration:

  1. Valid PA photo identification with your current address

    • If you’ve moved into PA within the past two months, we request proof you are obtaining a PA identification

    • If you are a PA resident and have moved within the last 15 days, we request a change of address card

    • If there are complications preventing you from obtaining a valid PA photo ID please submit the valid photo ID currently in your possession.

  2. Two proofs of residency (listing your name) one from each list:

    *Please note: Proofs from both list A and B require Guardian Name, District Address, and Current Date.

    • List A – Settlement Statement, Lease/Rental Agreement,  Real Estate Tax Bill


    • List B – Vehicle registration, current utility bill, paycheck stub with employer address and employee address, current voter registration, state/federal program enrollment, bank statement

  3. Child’s birth certificate

  4. Licensed physician’s documentation of immunizations displaying the student's name and date of birth or statement of religious exemption.  Please do not submit a photo screenshot of immunizations from your patient portal unless it includes the child's full name and DOB.  If your child's full name and date of birth is not clearly stated on the immunizations your enrollment will be denied.

  5. If applicable - Copy of Court Ordered Custody Agreement, IEP and/or 504 Plan

  6. If applicable- New Student Athletes (See the “Student Athlete” tab on the left to download and complete the applicable form)

Kindergarten Enrollment Information 

Your child must be age 5 by August 31 and have current record of PA required vaccinations to enroll for the upcoming school year. If you are unsure your child's immunizations are up to date, please contact your medical provider prior to submitting your child's enrollment.

Enrollment forms submitted for students who have not met the PA age and vaccination requirements will be denied.  Once your child has obtained the PA age and immunization requirements and you are able to attach the required records, you may open and submit a new enrollment.
